To arrange an appointment, you may call our telephone centers. We will make an effort to book an appointment at your convenience. If this is an urgent case, please, inform our front desk so that we may assist you as soon as possible. Prior to your appointment you will receive a courtesy call from our center to remind you the day and time of our meeting. You may also receive our questionnaire through the mail so that you can fill it out and bring it with you. If you have any questions or concerns, we will be more than happy to receive your call. We appreciate the opportunity to take care of you.
